Apply Of an Individual For Shifting to a New Family.

Required:

1) Already linked mobile with the Ration Card which you want to shift. Online Application will submit with this Number.
2) Any Ration Card No in which Family you want to Transfer.
3) Aadhaar Card/Marriage/Divorce Certificate /Other Suppl. Doc Upload File Should be 100kb

Goto https://food.wb.gov.in this link
Select “RATION CARD” Menu
Then Select “Apply Of an Individual For Shifting to a New Family”
Or Directly Go to This Link
Enter Mobile No. then Click “GET OTP”
Enter OTP The “PROCEED”

Then Select Apply For FORM-14

Steps: 3
Select The Member which you want to transfer
Then Enter the Card Number and Card type in which Family you want to Transfer.
Then “SEARCH”
Then “Your Details” and “Existing family members where applicants want to shift” Deatails will appear.
Then Click”NEXT”.
Skip the “UPDATE DETAILS” of family members Section.
Update the Address where you transfer the Card.
Give a Secondary phone Number or click same as primary or skip the section.
Select Relation with HOF (Head of Family).
Reason for shifting to a new family
1) Marriage
2) Divorce/Separation
3) Others

Upload supplimentary documents like Aadhaar Card/Voter Card/Marriage certificate/Divorce paper

Tick Mark The Consent
Then SEND OTP
Then Enter the OTP
Click Verify & Submit
